Step 1: Begin with a Preliminary Self-Assessment
Step 1 – Complete Your Online Intake
Start with our secure online form, which includes:
An ADHD symptom questionnaire developed by the World Health Organization.
This single form helps us see if you have symptoms that are shared with ADHD criteria.
Please note: This is a screening tool, not a diagnosis. It helps us decide if a full evaluation is appropriate.
Step 2: Medical History & Consent Forms
If your self-assessment suggests that ADHD may be present, we’ll invite you to:
Complete our detailed medical history form
Sign our consent for care.
Because your safety is our top priority, we review each patient’s information carefully to make sure our clinic is the right setting for your care.
Some medical or mental health conditions require a different type of evaluation or treatment than we provide here
If we find that our approach is not the best fit for your needs, we’ll let you know before scheduling the full evaluation
This step helps ensure that the care we provide is both safe and effective for every patient we see.
Step 3: Schedule Your Appointment
After approval of your forms, we approve your appointment date for your comprehensive ADHD evaluation.
Requirements:
Medical history & consent must be complete
$150 deposit (applied toward total cost of services)
Important:
Scheduling does not guarantee a diagnosis or prescription
Additional questionnaires will be sent before your visit to give us deeper insight into your symptoms and history. Please try to have these filled out prior to your appointment.
The evaluation’s purpose is to determine whether ADHD treatment is appropriate under our care guidelines
Step 4: Comprehensive Clinical Evaluation
During your 60-minute appointment, your provider will:
Review your questionnaires and medical history
Discuss how ADHD symptoms impact your daily life
Screen for other mental health conditions
Step 5: Feedback Session
In a separate follow-up visit, your provider will:
Share your diagnosis (or explain why ADHD was not diagnosed)
Clearly explain all findings
Present treatment options — including non-medication approaches
Discuss safety considerations and ongoing monitoring if treatment begins

Allows time for a thorough, unhurried review of your results
Ensures recommendations are safe, individualized, and evidence-based
Gives you space to ask questions and understand your plan
Step 6: Welcome to the Practice & Patient Portal Access
Once your treatment plan is confirmed:
If medication is recommended, your prescription will be sent electronically to the pharmacy of your choice.
You’ll receive full access to our secure patient portal, where you can:
Message your care team anytime (24/7 messaging — responses during business hours)
Download your medical records whenever you need them
Access educational materials and resources to support your ADHD care
Your patient portal is your personal hub for staying connected, informed, and in control of your care.
